Plants caulescent, usually branched from the base; stems slender, to 200 cm tall. Leaves about 24, to 30 cm long and 6 cm wide, crowded at the top of the stems, swor-like, recruved, grey-green, unspotted; margins with a white, cartilaginous toothed border; teeth less that 1 mm tall, 5-10 mm apart. Inflorescences to 50 cm tall, with 3-8 ascending branches; branches 5-6 cm long, flowers fairly densely packed; bracts 2-3 mm long, about 2 mm wide; pedicels about 9 mm long. Flowers dull red, nutant; perianth about 19 mm long, 5 mm wide acros the ovary, outer tepals free for 5-6 mm; anthers exserted 1-2 mm; ovaries about 4 mm long, 2.5 mm wide; stigmas exserted about 3 mm.
Aloe medishiana grows on limestone hillsides with other small shrubs and succulents at about 1800 m. It is konwn only from regions N2 and N3 of the Flora of Somalia.
